Standard Tort Claim Form 
It is required that every claim filed against a state or local agency, including school districts, be submitted on a Standard Tort Claim Form. The Standard Tort Claim Form is a special form for the claimant to submit a formal claim against a district. This form will be the claimant's opinion of how a loss occurred which may differ from the district's opinion. The district may create its own specific districtTort Claim Form, however, this form must comply with all components of the Standard Tort Claim Form. 


Local governments, including school districts, must make the standard form available with instructions on how the form is to be presented along with the name, address, and business hours of the agent authorized to receive the claim. 


WSRMP's Claims Department recommends that the Tort Claim Form be housed only in the District's Administration Office, and on the district website, and not distributed to the sites. Principals should direct any member of the public requesting a form to make a claim through the administrative office.
