South Whidbey Schools Issues employees the following federal payroll tax forms:
Form W-2 Wage and Tax Statement
Form 1095-C Employer-Provided Health Insurance Offer and Coverage
All federal tax forms will be posted in Skyward Employee Access annually by the deadline required by the IRS for each form. Deadlines for the current tax year can be found on www.irs.gov
W-2 Wage and Tax Statement
If you did not elect to receive your W-2 form electronically, the W-2 form will be mailed to the address on file with Human Resources.
1095-C Employer-Provided Health Insurance Offer and Coverage
If you were covered with health insurance through South Whidbey Public School benefits at any time in the most recent completed tax year, your 1095-C form will be mailed to the address on file with Human Resources. Starting in 2026 for tax year 2025, the 1095-C form will no longer be mailed unless you request a paper copy each year. Please note the 1095-C form is not required for individual income tax filing. If you would like a paper copy of your 1095-C form, you may send a request the address below. Requests will be accepted after the required posting date determined annually by the IRS and will be completed within 30 days of the receipt of the request. Please be prepared to authenticate your identity with any requests for paper tax forms.
