Final Forms Playbook (PDF)

We are very excited to announce that the South Whidbey School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

Please review the Parent Playbook (link) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”. If any questions arise, please contact our  Athletics Secretary, at 360-221-6808 ext 5423. 

 We are asking that ALL parents of athletes use FinalForms. 

Please register at:  https://southwhidbey-wa.finalforms.com and follow the prompts to create your account, create your students and sign your forms. Any physical forms can be dropped off at the ASB/Athletics office as usual.

Volunteer Coaches:

Volunteer application forms are on our website linked here: https://www.sw.wednet.edu/get-involved/volunteers 

There is a complete printable packet: https://www.sw.wednet.edu/fs/resource-manager/view/6ba557b4-f584-426e-9387-494fa20e6478 , 

OR  you can complete the digital volunteer form and simply print out the  WA sexual misconduct disclosure release form.

Volunteer Coaches must also complete FINGERPRINTING.  Island County Sheriff's office has the fingerprint cards. When you call to make an appointment, confirm that they have the cards.  If they do not, there are fingerprint cards at the district office-  South Campus.

Once you complete the forms and fingerprinting, bring the printed forms and completed fingerprint card with a check made out to OSPI for $50.00 to the district office at 5476 Maxwelton Road, Langley (the South Campus).   If there are any questions, please email info@sw.wednet.edu.
